JSPA is a skilled nursing facility located in Neptune, New Jersey just off Route 18 and down the block from Jersey Shore University Medical Center.
JSPA offers short-term rehabilitation, including physical therapy, occupational therapy, and speech therapy as well as long-term care services and a peritoneal dialysis program. We offer a beautifully renovated facility with the feel of a luxury hotel as well as the comfort of home.
We accept Medicare, Medicaid and most insurance plans.
Subacute
Seven days a week, an interdisciplinary team works together with physicians to create and implement customized rehabilitation plans for each patient. All residents are also monitored by a board-certified staff psychiatrist.

Dr. Suhas R. Patel, MD
Jersey Shore Post Acute is very honored to have Dr. Suhas R. Patel, MD as our medical director. Dr. Patel has been specializing in both Geriatric and Internal medicine for over 20 years.
Dr. Patel is affiliated with Jersey Shore University Hospital, Ocean Medical Center and Community Hospital and has been running his own internal medicine practice in Ocean, NJ for the past 18 years. Dr. Patel has a stellar reputation and is extremely well respected in our community by fellow physicians and specialists in our area who have had the privilege to know and work with him.

Under the leadership of Elmer Sadiang-abay, DNP
JSPA is very fortunate to have Elmer Sadiang-Abay, DNP as the consulting NP in our Nephrology department. Elmer has a Masters and APN degree from Monmouth University and earned his Doctorate in Nursing Practice at Walden University. Elmer is a dedicated, driven, empathetic and compassionate health care professional with a wonderful bedside manner and is a true patient advocate. Elmer is an important and integral part of our in house Peritoneal Dialysis and HD Programs and we are grateful to have him as part of our dynamic and progressive team.
PULMONOLOGY SERVICES
Under the leadership of Mona Awad, M.D. Pulmonologist
Jersey Shore Post Acute Rehabilitation and Nursing is excited to have Dr. Mona Awad, MD as our consulting pulmonologist. Dr. Awad is board certified in pulmonary and critical care medicine. She has been practicing for over 20 years and is affiliated with Bayshore, Old Bridge, and Riverview hospitals. Her team specializes in all diseases of the pulmonary system including COPD, asthma, bronchitis, cough and COVID-19.
She is joined by colleague Sarah Valvano, PA-C, a local Monmouth University graduate and board certified physician assistant.
Dr. Awad’s primary practice is located at 1 Bethany Rd, Suite 85 Building 6 Hazlet, NJ, 732-264-5005.
CARDIOLOGY PROGRAM
Under the leadership of HAROON KHAN, MS, PA-C, CCDS, CEPS
Haroon Khan, MS, PA-C, CCDS, CEPS is a board certified cardiology/cardiac electrophysiology physician assistant. He received his undergraduate and graduate training at the Wagner College in Staten Island, New York. He has been providing care for cardiac patients at Shore Heart Group for 13 years.
Shore Heart Group is a premiere comprehensive cardiology practice providing care for patients across the Jersey Shore.

Respite
Sometimes a caregiver needs a brief respite from the day to day responsibilities of caring for a loved one, either to rejuvenate or to take a pre-planned vacation.
If you or your loved one needs a little additional temporary medical or nursing support, Jersey Shore Post Acute provides short term respite care on a daily, weekly and/or monthly basis. During a Respite stay, short-term patients receive the same high level of care as other patients, in a safe and secure environment cared for by skilled professionals.

Kitchen staff is seeking new ways to increase the quality and diversity of foods. The therapists are as fine a group of dedicated people as ive ever seen. I was enormously confident to be working with such a group. The facilities are excellent and being improved as we speak. The aids are some of the hardest working people I've seen in any therapeutic environment. The nursing staff is extremely fined tuned and always willing to answer a question. The entire community of people in this facility are tuned to assisting patients recoveries. On the lighter side of things, there is a man named michael who is willing to bring a glass of ice to any patient who wants one, in order to keep patients happily hydrated! This facility can be considered a wonderful family for its guests.
